Passport Application
Requirements for Passports
Passports are processed for NEW applicants only and are accepted by appointment only. To make an appointment, please call (330) 676-7567. Walk-ins are not available. Applications can be picked up at the 930 Overholt or downloaded through the passport website. Please fill out your application (print legibly or type) as completely as possible, using black ink only. You must have the following documentation when you bring in your passport application for processing:
- One Passport Photo: Pictures must meet requirements as noted on the reverse side of your application, no school pictures are allowed
- Evidence of Birth - Choose one - It will be returned with your passport (no photocopies allowed):
- Certified Copy of your Birth Certificate
- Naturalization Certificate
- Previous Passport
- Children under the age of 16 must submit a Certified Copy of their Birth Certificate regardless if they have been issued a previous passport
- Identification– Choose one:
- Drivers License (Must bring a copy, front and back, on the same side or on two separate sheets)
- Government I.D. Card (Kent State identification cards are not valid)
- All children must be present when applying for a passport
- Both parents must be present when applying for children under the age of 16, or fill out the statement of consent or special circumstances: issuance of a U.S. passport to a minor under the age of 16 form that can be picked up at our office or downloaded through the U.S. Department of State website (Please note, this form must be notarized)
- Parents signing for their child must bring a photocopy of their identification, front and back
- Note: If the identification being submitted is from a state other than Ohio, a second form of identification must be presented.
- Passport Fee for Passport Book- Check or Money Order Only, No Cash:
- Make checks payable to U.S. Department of State
- $130 - Age 16 and over (valid for 10 years) (Increase effective 12/27/21)
- $ 100 - Age 15 and under (valid for 5 years) (Increase effective 12/27/21)
- Passport Fee for Passport Card- Check or Money Order Only, No Cash:
- Make checks payable to U.S. Department of State
- $30 - Age 16 and over (Valid for 10 years)
- $15 - Age 15 and under (Valid for 5 years)
- Acceptance Agency Fee(to be paid when obtaining either a passport book or a passport card) - Check or money order only, no cash:
- $35 - For each applicant regardless of age
- Make checks payable to the City of Kent
Effective February 1, 2004, every minor child shall appear when applications are made for a passport, regardless of their age. Additionally, both parents or legal guardians must appear to make an application.
Important Information
- When paying by check, it must have a name and address embossed on it. Counter Checks or Starter Checks that do not have the name/address are not acceptable.
- All birth certificates must be certified (embossed, impressed, multi-colored, or raised seal) and have the parent and child’s full name; date and place of birth; was filed within one year of birth. Hospital birth certificates are not acceptable.
- All applicants must present a valid state identification and bring a photocopy of the front and back of said identification. If the identification is not from Ohio, secondary identification is required. Applicants must bring something that has as much of the following information: Person’s photo, full name, date of birth, and document issuance date. Credit cards may be used, and a photocopy is not required, but it must be produced when making an application.
